NEW SAFETY PROTOCOL
We will continue to monitor CDC guidelines and make changes as appropriate to ensure everyone's safety.
- If you have signs and symptoms of COVID-19 or believe you might have been in contact with someone who may have been exposed to it, we ask that you reschedule your eye examination.
- An optician from our office will call you before your exam date to confirm that you are not exhibiting signs of COVID-19.
- We ask that you come to your appointment by yourself. If you need someone to bring you, we ask that they wait in their vehicle or outside of the office.
- Please bring and wear your own mask that covers your nose down to your chin while in the office.
- Upon arrival, please wait in your vehicle until a member of our staff calls you in for your appointment to avoid close contact in our lobby and exam room.
- Upon entry, your temperature with a contactless thermometer will be taken to ensure you do not have a fever.
- In an effort to reduce contact using paper and pen, please fill out the online patient form which is found in the title page above labeled PATIENT FORMS, or a staff member will call to do it over the phone.
